By creating a My Thrive account through your care provider, you’ll gain access to personalized resources, tools, and reminders that help empower you to seek the care you need.
You’ll be asked about your health history the first time you log on. This creates a summary you can easily access and share with providers at any time during your care journey.
Once your account is activated, your care providers will also add information to your health profile that you can view
and share.
After completing your intake surveys, your custom care plan will be ready for you to explore. This includes a library of relevant educational resources, as well as a follow-up assessment schedule.
You’ll receive reminders to contact your care provider when it’s time to schedule appointments and conduct self-examinations.
